Rheumatology Associates, PLLC, welcomes the privilege of working with you to take care of your patients with rheumatic diseases. We see patients only by referral from other health care practitioners. Our goal is to provide state of the art care in a compassionate environment, while providing the referring practitioners timely consultations with precise communication. We welcome your calls for curbside questions and our office will do our best to accommodate your patients in an efficient manner. Thank you for entrusting us with the care of your patients.

Include all supporting information that pertains to the diagnosis. Keep in mind if the patient has seen a rheumatologist or dermatologist in the past, we must have those records.

Fax referral form and all records to (502) 897-1792.

When we receive all information required for a referral, we will then:

• Verify insurance
• Have the records reviewed by a physician
• Contact your patient within 2 weeks. Please advise your patient that we do ask for a credit card to secure their appointment. (If patient cancels within 48 hours, there will be a charge.)
• Notify you via fax regarding the patient’s appointment date and time
