On your first visit

Please arrive at least 15 minutes before your scheduled appointment time to complete the new patient registration process.

Here’s what you’ll need to bring with you…

  • Government-issued ID
  • Insurance card
  • Your copayment or payment toward your coinsurance or deductible
  • Referral from your physician, if applicable (this is required by insurances such as Medicare and Tricare)
  • Completed new patient forms

What to expect

During your first appointment, your physical therapist will…

  • Perform a physical evaluation to assess your current status
  • Develop a plan of care specific to your needs and rehab goals
  • Help you understand your condition
  • Tell you what to expect during your course of treatment

Your first appointment is the perfect time to get answers to any questions you have about your condition or your plan of care. Don’t hesitate to ask!

What to wear

We recommend patients wear comfortable clothing and athletic shoes to their first appointment.

Under 18?

Any patient under the age of 18 must be accompanied by a parent or legal guardian.

Payment policy

All copayments and payments toward your deductible and coinsurance are due at the time of service unless other arrangements have been made. Our practice is happy to accept most major insurances including Humana, Anthem, BCBS, Medicare, and Tricare. To confirm our participation with a specific insurance plan, please contact our office. As a courtesy, our office will contact your insurance to confirm your quoted benefits. Should you have a need to make other financial arrangements, please contact our office prior to your appointment.
